Your first visit to Larry H. Miller, DDS Dental Spa involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. Please bring your driver's license or another picture I.D. and your dental insurance card (if one is available) to your first appointment. Also, please come to your appointment 10-15 minutes early so that we can get your new file information entered into our computer. You can also save some time by printing out and completing the patient forms (shown below) in advance of your appointment. In addition, please let us know before the time of your appointment if you have current dental x-rays (taken within the last year).

What To Expect
Being well-prepared for your appointment will ensure that Dr. Miller has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. We look forward to your first visit.
We strive very hard to keep all of our appointments running right on time. However, occasionally we do run into emergency situations. We will make every effort to respect your valuable time. We only ask that you do the same for us and our other patients. Please give us at least 48 hours notice if you need to reschedule or cancel your appointment. This helps us to keep our costs down and benefits every one. We reserve the right to charge a fee of 20% of services for no-show appointments.
Payment is expected at the time of services. If you have dental insurance...we will gladly process your claim. We do ask that you pay your estimated portion (i.e. co-pay, deductible) when services are rendered. Every patient's insurance policy is different and it is beyond the ability of any dental office to know the benefits and limitations of every plan. Therefore, our office can never guarantee insurance coverage for any service provided. The responsibility for any difference between actual charges and what the insurance company pays rests with the patient. If you have any questions regarding your particular plan benefits, please contact your insurance plan provider or call the customer service number on your insurance card.
